CAMHS Clinical Liaison Practitioner
My work is focused around transitions for young people who are currently inpatients in a Specialist Eating Disorder Units and coming up to the age of 18 years.
I provide support when a young person requires transition to an adult Specialist Eating Disorder unit, I also am able to support with transitions from hospital to community teams and following discharge from hospital. I am also involved in transition from CAMHS to adult services.
I am able to offer time limited support to ensure smooth transitions and avoidance of deterioration at this crucial time when transitioning between services and teams.
